Since 2012, the My Health Record system has improved information sharing for healthcare providers and consumers. However, fragmented care remains a concern due to data-sharing barriers and difficulties accessing crucial information. Consumers also seek greater control over their health information.

  • Phase 1b: Directed Information Sharing (including consumer mediated exchange)

  • Phase 2: Discovered information exchange

  • Phase 3: Enhanced services such as digital baby books, approved wearables and devices enabling comprehensive views of overall well being.

Anticipated benefits include reduced duplication of effort, removal of manual paper-based communication methods such as the fax and enhanced data access control for both healthcare professionals and patients.
